Chronic Disease Management and Prevention Program
The Chronic Disease Managemnet Programme (CDM) aims to maintain and improve patient quality of life for those living with certain chronic conditions. It has been developed in line with best practice. The CDM programme is available to patients with a valid medical card over the age of 18 with one or more of the following conditions:
Diabetes
Asthma
COPD
Atrial Fibrilation
Certain Heart conditions
History of Stroke
Once enrolled in the programme you will be invited to attend the practice nurse and your GP and will receive a personalised care plan and two GP visits per year.

The Prevention Programme (PP) is aimed at medical card patients 45 or over who may be at risk from developing heart disease or diabetes. It consists of an annual screening appointment with the nurse and follow up with the GP.
